The County of Kaua‘i Agency on Elderly Affairs invites you to attend a Better Choices, Better Health workshop being held in Kekaha:

Chronic Disease Self-Management Workshop

Tuesdays from 1:30-4:00PM

Begins on 02/19/13

Kekaha Neighborhood Center

The workshop is for seniors 60 years and older and their caregivers. The workshop is a proven, self-management program and developed by Stanford University. It consists of six sessions held once a week for 2 1/2 hours each session. Workshop size is small, with between 10-16 persons attending, led by two trained facilitators.

The cost of the workshop is $12 for seniors 60 years and older, and their caregivers, which includes a companion book, “Living a Healthy Life with Chronic Conditions”, and a relaxation CD or cassette tape. Scholarships for seniors are available upon request. People younger than 60 years can attend the workshop for a $40 fee.

Preregistration is required.
